Member cities can file claims online, by fax or mail. To file a claim by fax or mail, print off from below and complete the appropriate forms.

Workers' Compensation Forms
Minnesota's Workers' Compensation Statute requires that employers provide employees with a copy of the Minnesota Workers' Compensation System Employee Information Sheet at the time the employee is given a copy of the First Report of Injury (FROI). Members should make copies of this information sheet and keep them with FROI supplies.
